Transition planning and checklist templates are now available

As you are no doubt aware, ACTHD and CSD, in partnership with ACTCOSS have jointly developed Transition Planning Templates and Checklists for use by both sector partners and government.

We are piloting these tools and resources across a number of commissioning cycles, and we will continue to refine them over time based on feedback and experiences.

We want to thank ACTCOSS for its involvement throughout the process as well as everyone who is supporting us in these pilots.

The transition planning templates and checklists will guide transition planning discussions and activities throughout all commissioning phases.

You can find the templates and checklists on the Tools, templates, and training webpage under the Discover, Strategies, Design, Invest, Deliver and Integrate sections.
