We are seeking the views of our sector partners on a draft transitions policy and framework.
Together, government and non-government organisations have informed the development of the Framework over the past year. Together, we identified what worked well and what has not. We have developed the draft Framework based on sector survey feedback provided in 2022, by seeking views on early drafts within government and testing a draft with sector leaders.
The first piece of the Framework is a draft policy that provides guidance on:
- The scope, goal, objectives and principles that apply to transitions
- The challenges involved in transitions and what an effective transition is like
- Types of transitions at a service, program and the whole system level
- What the policy means for communication, timing, funding, planning, implementation and monitoring of transitions.
The policy will be supported by more detailed procedures, tools and resources that support capability and a monitoring plan to ensure we learn more about the experience and impacts of transitions on sector sustainability, organisations, the workforce, and service users.
Transition pilots will get underway guided by the draft Human Service System Transitions Framework (the Framework).
This is continuous improvement in action.
You can share your views on the Framework and/or get involved in a transition pilot project that will also inform the final policy and process guidance.
Review the Human Service System Transition Framework in Policies for review and provide your views to CSDCommissioning@act.gov.au or HealthCommissioning@act.gov.au by 1 September.
